It’s time to post your OSHA Summary of Work-Related Injuries and Illnesses for 2011

If your company has more than 10 employees, OSHA requires that you keep a log of all work-related injuries and illnesses that occur each year. You also must post a summary of those injuries and illnesses in a location where your employees can review it. If your company has more than one location, OSHA requires a separate log and summary for each location. Some companies with 10 or fewer employees also may be subject to the requirement — if so, those companies will be informed in writing by OSHA or the Bureau of Labor Statistics.

  • Injuries and Illnesses Incident Report (OSHA Form 301) — you must complete this form within seven days after you are notified that a recordable work-related injury or illness has occurred. An injury or illness is recordable it if results in death, days away from work, restricted work or transfer to another job, medical treatment beyond first aid, or loss of consciousness. Injuries or illnesses without such results also are recordable if they are diagnosed by a physician or other licensed health care professional.
  • Log of Work-Related Injuries and Illnesses (OSHA Form 300) — include on this log each injury or illness that occurred in 2011 for which you complete Form 301.
  • Summary of Work-Related Injuries and Illnesses (OSHA Form 300A) — record the total injuries or illnesses on Form 301 on the summary and post it in a location where your employees can review it. You must post the summary from Feb. 1 through April 30. You must complete and post the summary even if no work related injuries or illnesses occurred during the year.
